Texas Immunization Requirements for 2010-2011 School Year.
The state legislature has added additional immunization requirements for the 2010-2011 school year. Students must present proof of immunization compliance at the time of enrollment according to Texas Administrative Code, Title 25 Health Services, Chapter 97, Subchapter B, Rules 97.61– 97.77. These laws may be seen in their entirety at http://www.dshs.state.tx.us/immunize/school/default.shtm. Students must have these required vaccinations or exemption waivers on file before they begin the school year. Information regarding provisional enrollment and exemptions may be obtained from the website above.
New requirements for school year are in bold type.
3 and 4 year-olds (Daycare, PPCD, and Pre-K)
4 doses of DTP/DTaP
3 doses of Polio
1 dose of MMR
3 doses of Hepatitis B
3 doses of HIB (one of which must be given on or after the 1st birthday) or 1 dose of HIB after 15 months of age through 4 years
1 dose of Varicella (on or after the 1st birthday) unless child has verification of previous chicken pox disease
1 dose of Pneumococcus on or after the 2nd birthday, if earlier series not completed
2 doses of Hepatitis A
Kindergarten
5 doses of DTP/DTaP (included one on or after the 4th birthday- unless the 4th dose was on or after the 4th birthday, then 4 doses)
4 doses of Polio (unless 3rd dose was on or after the 4th birthday, then 3 doses)
2 doses of MMR received on or after the 1st birthday and doses are at least 28 days apart
3 doses of Hepatitis B
2 doses of Varicella - unless child has verification of previous chicken pox disease
2 doses of Hepatitis A (1st dose on or after 1st birthday and 2nd dose no sooner than 6 months after the 1st dose)
First through Sixth Grades
4 doses of DTP/DTaP (including one on or after the 4th birthday) ~Students 7 years or older are compliant as long as 3 doses were given with one dose on or after the 4th birthday
4 doses of Polio (unless 3rd dose was on or after the 4th birthday, then 3 doses)
2 doses of MMR received on or after the 1st birthday and doses are at least 28 days apart
3 doses of Hepatitis B
1 dose of Varicella - unless child has verification of previous chicken pox disease
Seventh Grade
3 doses of DTP/DTaP (including one on or after the 4th birthday) 1 booster of Tdap if it has been at least five years since the last dose of Td. If 5 years have not passed, the booster is due when 5 years have passed from the last dose
4 doses of Polio (unless 3rd dose was on or after the 4th birthday, then 3 doses)
2 doses of MMR received on or after the 1st birthday and doses are at least 28 days apart
3 doses of Hepatitis B
2 doses of Varicella- unless child has verification of previous chicken pox disease
1 dose of meningococcal vaccine
Eighth through Twelfth Grades
3 doses of DTP/DTaP (including one on or after the 4th birthday) and one within the last 10 years
4 doses of Polio (unless 3rd dose was on or after the 4th birthday, then 3 doses)
2 doses of MMR received on or after the 1st birthday and doses are at least 28 days apart
3 doses of Hepatitis B
1 dose of Varicella-unless child has verification of previous chicken pox disease; 2 doses required if 1st dose given after age of thirteen
